This only happens when you have both fairies on you at once. It still revives you but the other fairy that healed you before needs to fly all the way over.

I honestly don't quite understand "at first" but I think I know what the bug meant lul.

Have a Fairy Ring equipped
Collect 2 Fairies.
Die, letting 1 revive you. The fairy stays where you died last.
The 2nd fairy flies away to who knows where.
Die again, the one that revived you earlier now flies all the way across the dungeon to reach you.

Here you go, saved a bit of a headache xd.
The real problem is both fairies are trying to revive you, I could assume the second fairy didn't get to fulfill its state to revive and just standing there, menacingly.
