Something is bugged out, I've requested a movie and I constantly get notifications that it was approved, like every day at around 05:00 am coinciding with the Media Availability Sync schedule.
The movie is requested in 4k and non-4k, it is available in 4k, but not in non-4k.

just triggered the "media availability sync" manually, and can confirm that this is the one that triggers this request to be "approved" everytime (even though it was already approved).
This also changes the "requested" timestamp.
