Item Missing while trading

A quick back story before I mentione what my guildmate and I experienced just recently while upgrading an utlimate weapon.

A few months ago, there were issues where my other guildmates lost ultimate accessories after it was made on flaris NPC. So what happened to them was 1 person bought fragments from my other guildmate so that he can convert it to an ultimate accessory. Then when trading it back to another person, suddenly the accessory disappears on both sides. At first we thought someone made a mistake or got scammed from using the same IGN. Both these guys are trusted in our guild. They we're in a party too, so the one who traded was certain he was clicking the right person. So that issue passed away and they had to settle it in fair terms. They even tried to reach out to mods and ask if they can investigate and give them the logs, unfortunately they got a reply that flyffu cant do such thing.

After a few weeks. Same scenario happened but on 2 different people. Someone bought SL Gpro to pierce a suit then after trading, the item went missing again! Another issue escalated in our guild trying to settle this and there were exchange of unpleasing words from each other since both of them can't figure it out and resolved that someone is scamming. Both these guys had no scamming issue in the past on our guild as well.

Both of these situation happend on flaris NPC.

Fast forward to yesterday.

I bought ultima orb and sl shining ori from my guildmate to convert my weapon to ultimate. Since we had previous knowledge of items missing, my guildmate decided to go live on discord so I can view all the process of making my weapon ultimate up to trading back my weapon.

And so he did. My weapon is now ultimate. I saw it live on discord and just when we're about to trade, I Input the amount of penya I need to pay for. And I saw it appear on his trade window, he then put my weapon on the trade window and pressed ok. I crammed because I can't see my weapon on my window, but it's there on his live video. So we cancelled and re-trade. The transaction was successful after retrading.

My question is, has anybody had the same experience as we did? I was shocked when it happened and unfortunately we didn't have any record of it since I crammed and told him to cancel the deal because I aint seeing my weapon.

I think this should be something mods should investigate

holy shtt, at least you could see the weapon is not there, but it's shocking that an item can be lost this way.
