I have YouTube Music Premium subscription and I followed the guide on GitHub. I have format m4a, --bitrate disable, --cookie-file cookies.txt (I named it that way, in the same folder as spotdl). No errors. What am I missing?

Never got it to work. Yes windows will show you that the file is 256 kbps, but as the developer noted, its just what Windows is telling you, the metadata, the file isn't actually the higher quality. Use a program Spek to check. Its like me taking a 720p video file and exporting it as 4k. Yes, windows will tell me it is 4k, but the actual file is not in 4k. I have yet to actually get this to work.
:: use Spek and check the before and after to confirm for yourself. check the files you have now and then check them. Remember to always convert the files to mp3. Using Spek, if it is in the 15 kHz range, then it is 128 kbps. If it is in the 20 kHz range, then it is 256 - 320 kpbs range.
