The option to choose difficulty does not appear in my Gameplay menu now although it did a few days ago. Here's a screenshot

Then it's probably one of your mods breaking the menu.

99 out of 100 times something breaks for no reason, it's a mod.

Actually I found that, if I chose settings from the Esc menu rather than exiting to the main menu, the difficulty choice was there 😁

Because it's not supposed to be in the main menu. Difficulty is by character, not game-wide.
