#All Out Attack Finisher glitch

Some weird glitch has happened twice in 1 hour when im about to kill a "high level" enemy with the all out attack. Is this a known glitch caused by a mod, or is it something different? Im running the game from R2 and here is mods I use

I havent heard of it. You could try disabling the mods one by one to narrow down the issue

Yeah I guess I could do that, it would just take some time since its not every time it happens

disable Simple EXIST Nuke

if that doesn't help try disabling the no more all out attack fade in
