bought the (helmet?) a long time ago -
and today in BS3 I got a sword lvl 0 (a white one, with even no options :o)

yes, that's possible.
They drop from +0 to +4.

Unless you have that helmet from the Lounge, (Hades Helm) then it is +1 to +4.
