This happened with a character that health fixed to 1, you can see all the items i had right as the bug happened in 3rd zone.
I have a video if it happening but cant upload it because of discord.
also bullets flying backwards is very confusing. but dont know what causes that.

I have gotten this bug multiple times. It does not matter how many hearts you had as long as you die in the 3rd zone(I only get this when I am in the 3rd zone, have not got it in any other).
For the bullets flying backwards I notice I only get it with the hour glass.

Yea, Hour Glass makes all projectiles move backwards when facing the left side, it works on melee's too
