top layer coming out horrible

every time I print and there is a flat top layer, it comes out really rough and sometimes has holes
I have 3 top and bottom layers with ironing turned off

Did you change the top bottom thickness?

And what print temp and speed?
Is the cooling on?

no I haven't changed the thickness, not sure why it's like that, I am printing at 200 degrees at 80% speed
and yes cooling is on
