There's not much to go off here.
Why can't you retrieve your loot?
You'll have to go to the location where you died (it's marked on the map); your loot should be there.
If it's not visible, hit Escape an press the 'retrieve loot' button (or something similarly named).
If you died in an instanced level (when fighting a Great Hunts boss), it's a slightly different story.
If you die in there, your loot should be put outside the arena in a loot bag.
And there's always the option to restore a backup file from before you died.
