#PIM role assignments removed with no requestor listed

All our PIM role assignments have been removed from users and groups (except for Global Admin), however in the resource audit there is no requestor listed (see screenshot) does anyone know how this may have occurred?

Thanks, it turns out it was an access review that was set up but not documented or actioned properly.
