#10th grade math: Self-image questionnaire help

My daughter in 10th grade is needing some participation in a Google Forms survey as part of her maths homework. It examines self-image of youths and adults in relation to social media. She is having difficulty reaching her target of submitted answers. If anyone reading this could help submit some answers for her it would be greatly appreciated -- and welcome to comment on the quality of the questions 🙂. She needs in hindsight consider if the questions were clear and targeted enough.
