This has happened to me a few times, enough so that I think I've narrowed it down: If you do a lot of changes with the architect desk and hit cancel, there's a chance you'll end up with ghost items that appear in the place they were moved to but the game treats them as not so. A game restart fixes it. In some cases you can still place other items on spots that appear empty but aren't actually, functionally deleting what was supposed to be there. had to replant some resin tap trees due to this bug as they were just outright deleted after this bug.

Turns out it's very easy to replicate. Seems like fences are primarily what doesn't revert, but seen it happen to grass and tiles/tilled soil too. If any of the item types that do not move back during a cancel are placed in a spot that another item correctly reverts back to, the un-reverted block becomes a ghost one. If it was moved into an actually empty spot it seems to be fine and can be interacted with. I imagine the other versions of this I've seen (items getting deleted etc) are further complications of the same underlying issue and that ensuring blocks actually do cancel back correctly will chain fix the entire series of issues.
