Game keeps freezing, even when setting graphics down

Hey, I need help please. I love playing the game but i find that if I tab out of the game, it freezes and I have to shut it down via Steam. Every 2 in-game days or so the game freezes the same (even when not tabbing out).

Generally I noticed that most of the crashes/freezes happen when I'm inactive (stop moving for a second or check my inventory)

I'm playing on steam on the newest version, medium graphics. Thank you!
