I have my heritage rank at 267/268 but I can't get it to go above that? What qualifies for Heritage Ranks and how do I get it to get above this odd number...

Heritage is measured by the Lake Temple.

Is it measured by how many complete offering you have?

Yes, complete bundles. You do not get points per offering, just when you complete a bundle.
