Yes, it dies eventually, generally within 3-4 days of the start of winter in my experience. You can replace it with more grass starter, but that starter will also die within 3-4 days.
Not sure in the 1.0 but before, from what I noticed, in winter, grass would just not regenerate (at least in the animal enclosure... the rest I had harvested for hay). At the beginning of winter there was still grass and unlike other seasons, it would not regenerate during the night. So eventually my animals ate all the grass outside. Had to plant again in spring.
Yes, sadly it does. So you can, over the year, sometimes Cut through the grass to gather hay. If you have a silly amount of Fiber left over, you can stuff it into the dehydrator and it becomes hay 😉
As MidnightButterfly suggested, you can cut down grass over the year to get hay for winter. The trick is to not cut all of it, just thin it out, so it can easily regenerate. Leave patches of it over several days. I usually go in a patch, cut, move 5 steps or so, cut again and so on. Wait a week or so and repeat. I'm on year 1, fall 20ish and I have about 1000 hay stored and I still haven't cut the remaining grass on the farm.
Oh and in spring, plant some starters every here and there around the farm. It will multiply in no time. That or have a dedicated area just for grass growing.
