How do you give an NPC a quest item without gifting it accidentally?

Honestly, how?! There seems no option but to gift it, and if you talk to them, nothing happens concerning the item.

Have the item in your hand and it should replace the gift option. Should say something like "give item to complete task/quest" or something like that.
