Do we have any knowledge of the logic behind spawning ladders? Is it predetermined on floor generation, after x stones or does each stone irregardless of location have a % chance? Or do specific stones have a higher % chance than others?

I exclusively mine small rocks when searching for ladder because it's easier on my stamina. Other than that I've noticed no discernable pattern, such as being close or far to the elevator for example.
