If I remember correctly, they said it will be left open/not be specified how exactly the child is added to the family, to include other possible options like adoption and IVF, so it's up to the player to imagine which way it happened.

Other games allow you to have same sex relationships in which case the kids get "adopted". Perhaps all kids will be considered adopted. Or perhaps they'll make it vague but cute like a stork dropping off a baby. As someone who is adopted myself and Supports LGBTQ rights, I'm fine with the idea of all kids being adopted
