Currently, accepting same sex marriage is a rule that forces this change on the entire game world. This... is a bad idea. Instead, it should be more dynamic, much like in real life. A Cultural practice and/or religious one.

That way a culture might eventually adopt this, where their religion previously forbade it (much like IRL with Christian countries) and one can have some places fully accepting, while other are not accepting at all.

This can even allow for greater stories - a homosexual couple might flee the country to elope, being chased by religious people trying to stone them or even just parents trying to force an arranged marriage on them and the like.

Aren't the same-sex doctrines supposed to take care of that already? Even with the rule turned on, same sex marriages can only be done if your faith accepts homosexual sex (which many faiths don't).
It would be nice though to have an additional degree of tolerance above accepted, to represent the difference between faiths that merely accept extramarital gay sex and relationships, and those that allow gay marriages as well

Not exactly. One can always elope regardless of faith, but only if the rule allows it. If the rule doesn't allow it, you can't no matter what. The rule shouldn't even be a rule frankly. It breaks the immersion and way all other similar "culture" traits work. We don't have a rule about whether or not polygamy is accepted, for example.

Also, the way the rule is now, allowing same sex marriages breaks achievements. Which is pretty ridiculous honestly.

Also, as per the rule (quoting word for word now) "Makes same sex relations fully accepted by all faiths." (Bold for emphasis mine.)

That's not dynamic. Which makes no sense in any way.

to me same-sex marriage should be a higher level of doctrine for the acceptance of homosexuality. Much like witchcraft, witchcraft can be accepted without being a virtue, but it can also be considered virtuous. There, instead of homosexuality being considered a virtue, marriage would be permitted. Whereas with a faith that accepts homosexuality, homosexuality is not a crime, but one cannot get married.
