Forgot the item name but like the eagle talon thing that makes your projectiles home, item doesnt work, I dont know about every floor but I did experience it in the sewers.

Same here, came to report this bug, it occurs everywhere. I even saved and quit to restart, it still didn't work

Doesnt help that most enemies in the sewer are small and agile, this item would have been the perfect match to handle them 😭
