I died from my own dmg... Liv Empyrea's Chaos Oath apply a miniscule amount of dodgeable dmg, i did not dodge because i didn't see a need to, then i died... even when my HP was full before.
No, i did not get hit by the boss.

To add on to that, when my teammates clear the stage, they got stuck on Ending animation screen, the stage was not cleared at all.
My team consist of Uniframe, Attacker and Amplifier (me in Screenshot)
I encountered this once before too, my team also had 1 Uniframe at that time.
