When i die in hardcore my tombstone sometimes disappears, keeping me from touching it to repair my gear. I think it happens only if i die a few more times in the attempt to get back to it but i'm not sure and it certainly doesn't happen everytime.
This last time I had 6 pieces of equipped legendary gear broken. It costs countless dust to optimize and it's completely ruined my build. I can't even fight cause my staff was my priest item and my fire item and it's caput. Interestingly, It still tells me how far away my tombstone is but when i get within 7 meters of it, instead of telling me again it just equips the item even though it's still broken.
Very frusterating and may cause me to have to quit the ggame.

you don’t need tombstone to repair, just need to be on the spot where you died
To quote the wiki
To repair an item, it must be taken back to the location where it was broken, such as the character's tombstone.
If a character with broken items is unable to reach its tombstone before the tombstone vanishes, the items can still be restored to working order at that spot! If a broken item is clicked on, it will display a value of how far away from the character's current point the item was broken.
