There are some actions that cause death with iocaine practice dose, but most of the time it's possible to respawn, or fix it by re-logging.
Sometimes though, we're stuck in death and cannot do anything until the debuff goes away in 5mins.

Working as designed... i.e. not a bug. Enjoy a lovely, extended dirt nap while leveling it 🙂
