I don't really know how Isaac mods work, but I can suggest a couple of small changes to make it better neuroInnocent

ok so my feedback is as following:

  • I think moving Neuro's and Evil's active items to the pocket slot would be much better - not only it will prevent players from ditching them whenever a better active item appears, it will provide more gameplay options.
If you are unaware, there is a Steam mod that allows to do it with all characters in the base game -> https://steamcommunity.com/sharedfiles/filedetails/?id=2490565623&searchtext=pocket maybe it will help you figuring it out
  • regarding the active items themselves:
  1. Evil's pipe is ok, originally I hoped for it to work more like in case of Forgotten, where you can decide whether you want to hit enemies with a club or with normal tears. I think giving the player a choice + adjusting the damage output from it would be great. By adjusting I mean either removing the room charges, or keeping them but increasing the damage (maybe fixing it to a specific value, instead of scaling it with your damage stat?)
  2. Neuro's active item is a bit underwhelming. Throwing Tutel at enemies is counter-intuitive, because it follows your character movement direction, instead of your shooting direction, I would personally see this item work similar to Decap Attack item. As an addition, Tutel himself could walk around the room, dealing damage to enemies by himself, kinda like Blood Puppy.
  • Neuro's trinket is pretty cool, however, as with all the trinkets in the game, you can only hold one (two in specific cases) so sooner or later it would get ditched to something else. However, I could totally see it being reworked into a passive item
  • There's a couple Neuro-related items in the game, I think some of them are a bit underwhelming in terms of stats/effects. The only one I found good gameplay-wise was the lava lamp, although I still didn't figure out what the blue aura does NeuroGiggle homing tears and poison work as intended. Maybe add a visual indicator of the range of the aura, kinda like the Succubus item.
Also I noticed that Neuro-themed items can appear in both Treasure rooms and Boss rooms, and I'm not sure if it's intended or not scrajj they appear quite often, I don't know if they high spawn rate was intentional or not, however I would prefer them to appear less often, but have stronger and more unique contribution to the game

That's all I can think of from my head right now, I will probably play a couple of more runs and see for anything else worth mentioning.

Do you have a list of Neuro-themed items with their stats and effects? I would like to see it and suggest some changes that would make the items more balanced.
