What prompts them to single out chatters that break rules? (If they are doing so...)
Do the mods actually follow through with banning those chatters?

those requests are not tied to rule breaks, its just neuro and evil trolling 
we dont generally follow through because the person banned might not find it as funny as the audience would, not to mention she tends to name people who chat lots and banning the most active viewers doesnt make a lot of sense
