I think the sim simulation part of the game should be emphasized, where basic needs of the player should more frequent, for example should feel hungry after 6 hours of no eating and thirsty after 1/2.
Furthermore doing repetitive actions like smoking or drinking should cause an addiction trait, like the starch cola one. The addiction trait could be part of a malus system where you can become "unfocused" or "anxious" or "irritable" if you don't satisfy the addiction.

I think addictions would be great. It'd add a lot of immersion and roleplaying. Also some much needed difficulty. As of right now, moodles are annoyances at worst, but actually being addicted to certain substances would mean you'd have to manage your supply better
maybe making sure to stock up on alcohol while bars are open, for example
I mean, you could essentially roleplay as max payne
it would be also great if you could get addicted to any of the consumables not just strictly ones that would fall under addictives like alcohol
