Yes, in theory, a health indicator (health HUD) can be implemented and would not be considered illegal as long as it doesn’t provide unfair advantages based on information the server doesn't already give you.

On those servers it could be blocked as I mentioned before, for example Freelook on Hypixel
The use on servers that are not blocked and are not a trap is up to the user.
Other clients have the life indicator and are not banned within them.
