I have three monitors: One big, one less so and my Cintiq. They are in exactly this order left to right.
Photoshop starts on the Cintiq, every un-anchored tool bar is on monitor one. I move everything to the Cintiq, sure, but then every popup confirmation dialogue is on monitor one... I move it all to monitor one, now the dialogues are on the Cintiq!
What the heck is going on?

Usually, everything appears on the first monitor. Change it in windows system settings. Then move Ps to the wanted monitor, then quit and re-launch?

In my experience with the same exact setup (3 monitors, cintiq on right), the popzups will always show on the screen you had them on last. AKA, your Layer Style window will pop up on screen 1 until you move it over to the cintiq. Next time it will pop-up on the cintiq. Every pop-up screen has to be moved individually though
(there may be a way to change this, but Ive just been doing it manually)

Moved at least once (to the desired screen) or every time(it will always pop up on monitor one on restart)?

For me, once I move the pop-up to my Cintiq and close it there, it has stayed there even after restart (but if you move photoshop to Monitor 1, the popup will still be on the Cintiq where it was last)
