This magazine insertion indicator icon can get stuck.
Steps to reproduce: Hover magazine over mag pouch for the icon to appear, then, without despawning the icon, insert the magazine in a gun. The icon will stay there indefinitely until "refreshed" (meaning a new mag is hovered over the pouch, which re-runs the UI icon script, properly despawning as intended it once the mag is moved away) or until a mag is inserted.
This behavior has only been observed and tested with the black, large pouches on the heavy vest, and only observed with 30 round full black AK-74 mags and 33 round, full glock mags, all with tracer ammo, but might extend to other magazines and pouches.
