This isn't a bug per se but when you have index controllers at least the zone for the mag to register it will load into the gun is so small or low that it prevents you from easily inserting it. It often takes several tries to find the right spot to do it as the top of the controller sorta blocks you from getting it in the right spot easily.
This appears to be new to the beta.

I am having this happen with the new beta 2 as well and I have a Quest 3/Pro controllers. It can be a bit painful when fragments are coming at you in pairs and you can't load the gun fast enough. 🙃 The sweet spot seems to be a bit further out from the end of the gun for the PM, GSH and Glock (so far) and it takes a couple seconds to register.

I am not sure if a fix went in, if I learned the sweet spot or if the extended mag for the glock loads easier than the shorter ones, but tonight I was able to load the extended glock mag a lot easier. I tried it at the base, forest and new map.

This was happening to me this night.
From the start of EA my controllers would hit together by the time I got the pm mag in, but that was the only gun.
Tonight I had the gsh doing it really bad and did find that moving the mag away would get it to slide in.

i figured out that "re-grabbing" the pistol fixes the mag inserting
