i've been having issues with it ever since it was added this wipe. i found out your fps goes back to normal after you shoot a bullet. so lets say your normal fps is like 70, but you're running 20-30 all the time, if you shoot once, the fps will go back to normal for a small amount of time. im guessing the faceshield continuously calling new sound files fixes the fps decrease binaural audio causes (the same way the bullet fixes it)

For starters make sure your drivers/BIOS is up to date, namely check your GPU drivers (e.x. Nvidia uses GeForce), consider doing a clean driver install (although if there is no glaring issues this shouldn't be necessary). For all your other drivers, if you want to be lazy download ccleaner and use the pro trial, comes with an automatic driver updater. People say that's dumb and cringe but it's better than all the other shoddy automatic driver updaters that definitely are not sketchy.

In your launcher, clear your cache and verify game integrity.

For game settings, try using "Only use physical cores" - this will more than likely do nothing or it will help a lot. Test it for a few raids, if it changes nothing just leave it unchecked then.
Bump your graphics to high and just try it - or at minimum on medium. Putting settings on low puts more load on CPU which Tarkov intensively fucks over.
On massive maps (lighthouse/streets) consider turning your postfx off
