Overall you just go in and try to reach the squishiest target you can find and soaking as much damage as posible

Your team has to damage the enemies while you do this, hopefully low enough for you to secure a kill with ult

If they don't, you're not comunicating effectively (use pings) or they're bad (in this case you're better off splitpushing, buy Hullbreaker and Deadman's Plate, go to an abandoned lane and smash as much tower and minions as you can then run away or kill whoever they send for you)
