Turns out if you dash when you are supposed to be dragged up to heaven, it doesn't. It lets you keep going up and up. There is also no death plane so you can just escape the map (which gets you softlocked till quitting). You can also get stuck in the walls up there which if you unstick yourself with the rocket launcher, it will get the satchel stuck.

Turns out to trigger it you have to die/have a satchel near/in the trigger zone for heaven, then the trigger stops working all together
Looks like it has to be on you first satchel (maybe) can't pull it off after one decide to noped out of bounds

I had the exact same thing happen to me on my first playthrough.
