Forfeit not working properly

Clips don't work so all I can add is a screenshot.
Both remaining players voted for it but it didn't forfeit.
My theory is that it was just very improbable timing of the 3rd player leaving causing it to not work.

happened again; presumably the 3rd player leaves after it has checked how many players remain in the team but before it shows the forfeit popup its strange (3rd player left during goal replay, popup came in somewhere during faceoff I think)

maybe its just my presence causing it though idk
