I discovered it when i think i used a stamina potion then gone to the chest menue swapped chests witouth opening them, so it seems u need to use feathers/drinks/tokens and be in that menu so far i wasnt able to recreate this there seems to be one extra step involved

Managed to see this twice using a drink to heal polonia on tower. Force killing the game resets it
