It sounds like a distorted guitar but it's hard to tell if there's something more happening here

that's distorted guitar, yes. The weird overtones (that high "edge" that comes in partway through) are called pinch harmonics and you get them with a special playing technique where you lightly touch the string while it's already vibrating. Aggressive distortion/overdrive makes them a lot stronger, but getting the technique right still takes a decent bit of practice. There are lots and lots of tutorials on YT to get the basic idea.
