What I've heard most often is that he became lazy, ignoring his duties, until the Flood eventually amassed a Gravemind and captured him.

Rampancy might've been implied, Spark's condition gradually deteriorated over the course of the CEA Terminals, especially after losing contact with the other Monitors. Something similar likely happened to Tangent.

And it eventually fell to Automated Sentinels to set up the Quarantine Zone.

If your just a normal Marine or a Sergeant, then you aren't required to have them. Therefore, you can't get them at all

Something I'd be interested in is a person who has a hacked neural chip and is capable of hacking into certain UNSC/ONI devices without being detected to be honest.
