Chaos can sometimes throw it off. As well as Styx chambers.
In case you're unaware, you can check what room you're in by looking at the top right corner of the screen when toggling the option to check what boons you have.
Just check it after beating each boss and you'll see a consistent pattern.

Things I've noticed that made me reach that conclusion but still thinking about it :
- Skelly is a skeleton.
- Hades is throwing skeleton heads as bombs in the fight.
- Skelly wants Zagreus to get strong.
- Hades wants you to stay and tries to demotivate you of going out. - What if he pretends and hiding his true feelings?
- Hades is the typical Dad character we see in cliche shows like "You are a worthless son. - But you are my son, I'll help you." sort of thing. Tsundere, mabe.
- Skelly is calling you Boyo
- Hades is calling you Boy
- Skelly is like a brother-mysterious figure that supports you and doesn't say where he comes from or who sent him.
- Hades is pretending to be Skelly or sending part of his soul to be one since he is on the throne.
- When Hades is missing from his throne, Skelly is also missing from your room.

I've noticed no hints on what time period it is, but some of the joke endings reference carriages as standard transport and I like to think those still adhere to some general time frame relevant to the game
