The final boss in the 4th biome will be Cerberus, and you can't find the will in yourself to hurt him, so you kill yourself instead. The whole gameplay loop is one day you hope Cerberus won't be at the end.

he gets up there, goes back in to bring achilles and patroclus, and they look out to the horizon

achilles and patroclus sit down against a marble wall, chatting about what they will do now
offhandedly, patroclus recalls the time sisyphus attempted to escape death, and how he was brought back to the underworld
achilles looks down at his chest, and sees that tisiphone has followed them, her whip piercing straight through his ghostly chest
before he is brought back to the House, achilles utters one last statement to patroclus:

Picture it: Zag is walking up the stairs to the Doors of Death (Entrance to the Overworld), when suddenly a ghostly scythe appears and puts Zag in the position he is before he dies (he's ragdolled up when a pool of blood circles beneath him). This time, instead of falling into the pool of blood, there is some dialogue between Zag and Than, where Than says that he was forced to do this on his duty, but instead of instantly killing Zag he will challenge him to fight. Then the screen slowly fades out from black as he is launched down the stairs and into a battlefield. The fight begins.

I had Poseidon/Zeus/Artemis and Zeus offered me both duos, I took Poseidon's and then in Elysium I got a boon from Zeus and Artemis and neither offered Freak Accident
And I'm p sure duos have less rng than the other boons because you almost always get them once you meet the requirements in my experience

I think those are generally referred to as "remembrances" and are things Hades said to Zag in the past. Some give some little information about certein character backgrounds, etc. Some might also have little spoilers in them how the game will develop (for example Hades implying ||that he himself tried and failed to escape the underworld||):

wow the rapid fire cast from Hermes felt really good. Had 170 dmg Athena-cast, then 300 dmg from dislodging Artemis boon and that the cast ammo dislodges in 6 seconds. Pretty fast bosses
