Which fleet is your boss, fleet, and where is your third vanguard unit for fleet 2?
Vanguard dying in 12-4 on the boss generally means you are lacking in damage and so the fight is taking too long.
Are you clearing initially on lethal?
Your BB should be flag. Screenshot your gears on your ships and post them here. The fleet comp you have isn't very good but their levels alone should be enough if you have decent gear on them.
Unfortunately not, you have to go ship by ship. That one ship alone tells me gear is likely an issue on all your ships though, that mg is very bad.

You can refer to this for basic gearing. 12-4 boss has medium armor, so you want a gun that does good damage to medium,.
As for Musashi, she will want this gun

Yep, big issues there. That fighter should never be used, the BTDs are inferior to helldivers.
I was using the KMS fighter for grafs ability
You'll need to craft a plane in gear lab likely to get a good one to meet her reqs.

For fighters the purple f6f hellcat is probably best of those not already in use.
Or you could take the gold one from Enty, since she's not in your fleets.
so switch one of the Helldivers for a Ju-87

what changes do I need to make to the vanguard?
Eugen could use an anti-torp bulge (purple aux) and whatever EVA aux you can get. Otto double HP auxes, Regen an HP aux and an EVA aux.
SS their gear and let me see their setup currently
Drop the oxytorp on otto for an hp aux, give Regen an AA gun more meant for damage (that one you have is mainly a stat stick for HIT/Accelerator for main fleet), and take off the toolkit on eugen for a torp bulge. The guns on them are fine, though Otto could definitely use a better one if you have one.

General rule of thumb, DDs get double HP auxes, CAs get EVA, and CLs get HP/EVA. You're trying to augment their weaknesses, not boost what they're already good at.
Otto is rather tanky for a DD so she can get away with other stuff, but since you're dying you want more surviability.
Is tool kit and SG radar good on regens
Yes, that will work.
These would be better than SG radar though, if you have them.
Double shells are fine for now on the BBs.

Yes. W13 gets heavy on AA requirements, W14+ has very specific fleet comps needed. W12 is the last world you can kind of just use whatever as long as it is leveled with decent gear.
You can worry about that later though, you'll have plenty of time to level up ships/farm gear while grinding w12 on safe difficulties.

You do everythingin opsi pretty much. Follow the story until you clear chapter 4, and buy out as much of the rare stuff from the shops as you can before the month resets.
Material reqs aren't bad for gear lab, but you do need to regularly do operation siren to get them, as it is the main source.
