#Carnage in C-minor achievement not popping

I'm on PC/Steam, came back to play and finish 100% the game and I got the Bolt Driver weapon that drops when you do the correct pattern but no achievement.
Have tried relogging multi times and re-entering it but no dice.

na it is still bugged. I ran 3 adventure modes, 2 campaigns, reinstalled the game and created a new character. Always played the song, always spoke to the fleutist. Got boltdriver everytime, but never the achievement

and the fleutist never had other dialogue options
