2011 police spec crown Vic 6.4L V8 fuel flex

So my car has eaten through an entire bottle of coolant in two weeks and is displaying these codes (image below) Which online, it's telling me my cylinders are over heating. Plus it's displaying low liquids even though they are all all full.

I believe the problem was solved I took it to a shop and they are replacing my radiator cap

It wasn't leaking but when I was driving it would just boil out
