It's hard to pinpoint the reason this is happening.
Based on Brazilian servers and players, I'd say that the lag spikes in Milan servers are more likely to be a regional infrastructure problem (they are obviously using the exact same server binaries and cloud service provider - AWS).
I believe that one or more of these problems are the reason:
Local ISP Quality: This can affect routes from your ISP to the game server, causing latency increase, or package reliability could be low.
Local internet infrastructure: Taking a look at a few Global Internet Index / Rankings sources, I noticed that Brazil is slightly higher in internet speeds compared to Italy. **But most importantly: ** Brazil's fiber adoption is way higher than Italy's, which is probably a huge deal and can explain some differences.

Network Congestion: While Brazil alone has a comparable population to Western Europe, network usage is probably way higher in Europe and could cause network congestion issues at specific time windows.
Higher traffic on AWS Milan Data Center: While I don't think this is one of the issues, we can't completely disregard the possibility of AWS Milan not being able to handle all of the traffic in peak usage times.
