Just came across this. After clearing Stage 11-18 normal, the CG played through twice for some reason.
First time it finished playing it cut back to the character as if it was going to show the battle results, but instead it moved back into the Cyberangel: ZERO Exception CG again.

Whenever her summon lands a hit the game freezes.
After a few seconds the game shows the error message, when resumed the summoned Knight model freezes in place, this keeps happening till there are several models in stage.

Recently, I got a fire damage Stigmata to replace her, so I did and... I don't feel like her attacks got any slower, but I feel the attack speed difference a lot if I equip the stigmata on my Sixth Serenade. Such cases have been repeated many times and the answer is simple: Check your Internet connection to the servers. If you are unable to connect,use VPN if the connection is interrupted. And it is interrupted at you because in your Internet provider there is a firewall which does not allow to give access to transition to these or those resources

This is a recurring bug I noticed to happen almost every day now, it seems to revert to some earlier CH version, before the introduction of the twins, so any character implemented after this "snapshot" of a version doesn't work and just have the default idle animation. If you have both the twins out on the bridge, this bug also causes them to "merge"
I noticed that this is fixed if you go to the dorm or do a mission.
