Issue 1: Within the section provided, I think that there is no suggestion of the theme being a city. When I think of a city I think of skyscrapers and areas densely populated with buildings. So the first thing I would do is create a background that has multiple layers of buildings, that moves in a parallax to make the level feel more city-like.

Issue 2: I think the idea of a 4 minute level is too broad and could lead to the level becoming boring without proper planning. To fix this I would write a more detailed plan of the sections and type of gameplay I want in those sections. For example I would say I want the first section of the level to be more focused on building the atmosphere of the level with detailed block design, meanwhile later in the level I would focus more on effects to simulate the busyness of the city
