There is a spot in the third room where I fell down and couldn't get out so i'm just stuck for the rest of the match or restart. I don't know if its a simple as putting a pipe there or if its meant to be like this.

As a general rule, don't OOB unless you know it's safe or you have one of the modded unstuck tools
because... well, these are everywhere
Assuming this is an accident you can gamble with host migration and see if you can load a previous save and teleport out
Otherwise you'll never get out

Some people report stuck spots where you'd only get to with out of bounds stuff, so i just put that out there whenever I see stuff like this
There are some modded stuff that can help prevent this stuff, but nothing basegame will save you

here is a screenshot of it in debug with the offending geometry object highlighted. I am aware of this one but there is no modded fix at this time-- but I am collecting a repository of all known softlocks, currently have over 10 of them
R1C1 has only this one
You thankfully shouldn't see another for awhile, and most are very hard to find but the most likely one you are to encounter is in R5D2

One more thing: that fix only works if everybody in the lobby runs it (can turn off MTFO to re-enable achievements, still works).
