yes I've failed a mission a couple times as a 4 player team
i havent tried failing with 3 bots

If you check your log file in appdata/10 chambers collective/gtfo do you see the following text?
Achievement invalid, must be a full team (Humans or Bots) ?

Then the game detected that there was an empty slot when the expedition failed.

thats strange, yesterday I wiped with 4 players, I cant remember exactly but is it possible that someone alt f4'd before going back to the lobby and that forfeited it?

Should be easy to just start a lobby of yourself and 3 bots, drop into level like a D or E teir, and just wake everything up and just let the enemies kill you

That should not impact this achievement, it operates differently than those that require a synced state.

Are you talking about the same achievement? This one is about 4 people dying. You're talking about succeeding with 4 people never going down (and that one does have a synced state that can get broken, most easily worked around by starting fresh like you said. That will be fixed in the next patch)
