my friend got pushed outside of the map by a swarm of eyeball creatures, and he had to kill himself after i solo'd the rest of the level

Had the same issue happen. Same level, swarm of eyeballs pushed me out of the regular arena. Could move around but couldnt leave the small area I was in.
Looks like both myself and LeShorin's friend had the flashlight trinket. Might be related.

in unity at least the fix for this is to make sure the engine uses the video card for collision updates, so you move functionality to fixedupdate() which is aligned with the FPS instead
