if you're wanting more frames, then go with the lower options; if you're wanting better graphics and don't really care too much about frames, then go with the higher options (ultra isn't really necessary to use since it sets all settings to its max, even the small things that you probably won't notice)
and yeah, medium and medium-high are good middle grounds
and also, if you're not satisfied with the settings that you get with a certain preset, you can change the settings that the preset sets through the modules system: https://docs.mastercomfig.com/latest/customization/modules/
that would allow you to pick something like the low preset and still be able to have some certain settings bumped up high
